A Senior Care Manager can assist clients and their families with many common care situations including:

Medication setup and management
Skilled nurse visit
Accompany client to the doctor and attend visit
Facilitate/prepare documents for provider(s)
Arrange DME (durable medical equipment) setups
Advocate for client, per family request
Schedule calls to check on client, per family request

I have now used SHC every day for over a year and couldn't be more pleased with the professionalism, responsiveness, trustworthiness, and compassion through some very difficult times. SHC caregivers have provided care at home including 24/7 hospice care for my relative as well as care in the hospital, rehab center and for visits to the doctor. I trust them implicitly. I made appointments with three agencies and can honestly say choosing SHC was the right and only choice. The other two agencies didn't begin to compare to what SHC offered and the way I viewed SHC. In May, it appeared, my own dad needed services and I called to have him evaluated. A nurse came the next morning and determined my dad needed hospitalization. I called SHC again and they were there to welcome my dad back at his home and were there with him 24/7 until he passed away.
